On behalf of Qatar Airways, we wish to express our sincere apologies for any inconvenience you may have experienced during your recent travel with Qatar Airways.
We wish to advise that in the unlikely event of loss/delay or damage of your checked baggage, the airline's liability is limited in accordance to applicable International Convention Regulations and Qatar Airways Condition of Carriage. However, any such claim is subject to proof of loss, e,g. purchase receipts or proof of ownership.

After careful examination of your claim, Qatar Airways will be offering you the below settlement against your claim.

Kindly complete valid Beneficiary Bank details if you agree to receive the payment as full and final settlement. Furthermore, you are kindly requested to complete the attached ‘Release and Discharge’ form. Additionally kindly complete ‘Authority Letter’ If you are authorizing someone else to receive the payment on your behalf.
In accordance with U.S. Department of Transportation regulations, Qatar Airways will refund the excess baggage fee/s you have paid in respect of the item/s which were not carried on your flight/s. Please complete the form below with full details of the bank account to which you wish the Excess Baggage Refund to be paid. Should you wish the Excess Baggage Refund to be paid to an account held in the name of a person other than the passenger under whose name the relevant excess baggage was checked-in, please also complete the 'Authority Letter'.
